1. Confidentiality of Health Information

4.6 Pursuant to Authorizing Court Order

We may disclose your protected health information pursuant to a court order that follows specific application procedures to protect your identity and makes certain determinations as outlined in federal regulations.

We reserve the right to contact you, in a manner permitted by law, with appointment reminders or information about treatment alternatives and health-related benefits appropriate for you.

4.11 Other Uses and Disclosures

Other uses and disclosures not covered by this Notice will be made only with your written authorization or that of your legal representative. You may revoke any authorization at any time, except to the extent we have already taken action relying on it.

5. Your Rights Regarding Your Health Information

5.1 Right to Inspect and Copy

You have the right to inspect and receive copies of your health information that we maintain about you. In limited circumstances authorized by law, we may deny your request. You will be notified of any denial. Some denials are reviewable, and you will be notified of review procedures. Other denials are not reviewable by law.

5.2 Right to Amend Your Health Information

If you believe your protected health information is incorrect or incomplete, you may request an amendment. We may deny your request if it is not in writing, lacks supporting reasons, or if we believe the information is:

• Accurate and complete
• Not created by us (unless the creator is no longer available)
• Not part of the information we maintain
• Not part of information you would be permitted to inspect and copy

If denied, you will receive notification and instructions on submitting a written statement of disagreement and requesting that your amendment request and our denial be maintained with your records.

5.3 Right to an Accounting of Disclosures

You have the right to receive a list of certain disclosures we have made of your protected health information. This list will not include disclosures made for treatment, payment, or health care operations; pursuant to your authorization; or made directly to you.

5.4 Right to Request Restrictions

You may request restrictions on the protected health information we use or disclose about your treatment, payment, or health care operations. While we will consider your request, we are not required to agree to it. If we agree, we will comply except in emergency situations where the information is needed for emergency treatment. We will not agree to restrictions on uses or disclosures that are legally required or which we reasonably believe are in your best health interest.

5.5 Right to Request Confidential Communications

You may request that we communicate with you about your protected health information in a specific way or at a specific location (e.g., only contact you at work or by mail). We will accommodate all reasonable requests.

5.6 Right to File a Complaint

Violation of federal law and regulations is a crime. Suspected violations may be reported to appropriate authorities. If you believe your privacy rights have been violated, you may:

• Contact our HIPAA Privacy Officer in person or by mail at the address above
• File a written complaint with the U.S. Department of Health and Human Services

You will not be penalized or retaliated against for filing a complaint.

5.7 Right to Receive a Copy

You have the right to obtain a copy of this Notice.

6. Changes to This Notice

SoCal Traditional Living LLC DBA Purposes Recovery, reserves the right to change the terms of this Notice at any time. We reserve the right to make the revised notice effective for protected health information we already have about you as well as any information we receive in the future. We will post a copy of the current Notice on our website. The Notice will contain an effective date.
